The test quagmire is, without competition, on the top of my mind. The stalemate between Government and the regions seems total. The regions seem to have dug into a non possumus position, and refuse to move.

For a moment, I hesitated. Are tests of low value? Then I realized that even a layman could understand that they are of paramount importance. In the successful cases of Iceland and South Korea, plenty of testing have been done. Most people saying something about fighting the virus underline the importance of tests.

I also reflected on the fact that tests give us statistics for future research. In a society which takes an interest in science, we right now ought to collect all data we can.

The situation seems strange. Perhaps we need a State Psychologist as much as a State Epidemiologist?

Homes for elderly
Tegnell is opening up for visits in homes for elderly. (Anna-Cajsa Torkelsson, Omni, Idag 12:24)

This puts one question at the fore: Shall visitors and the elderly carry face masks?

In Sweden, we seem to disregard de face mask question, and the information about them is non-existent. Everyone has his/her own idea about how they work. One can, however, see more and more people wearing them.

Finland
Text-TV today reports that Finland is going to spend about one billion krona on rebuilding their supplies for a new wave of Corona. Will something similar happen in Sweden? My guess is unfortunately no.

Other
The matter of the employment safety law is now making the head-lines. Most unfortunate timing as I see it. We could have an electronic national referendum on the matter this fall, and thus temporarily take the matter off the agenda.

SWEDISH

Testhärdsmältan ligger i särklass högst i mitt medvetande. Låsningen mellan staten och regionerna tycks vara total. Regionerna verkar ha grävt ned sig i en non possumus position, och vägrar röra sig.

Jag drabbades en stund av tvekan. Kan det vara så att tester inte är bra? Insåg sedan att till och med en lekman kan förstå att de är viktiga. Det framgångsrika Island tar testat massor, likaså Sydkorea. De flesta som uttalar sig om smittan verkar vara eniga om att tester är en av nycklarna. Jag tänkte vidare att tester är det enda som kan ge oss statistik för framtida forskning. I ett samhälle som är intresserat av forskning borde vi samla data allt vad i orkar just nu. Situationen verkar märklig. Vi kanske behöver en statspsykolog lika mycket som en statsepidemiolog?

Äldreboenden

Tegnell öppnar för lättnader i besök på äldreboenden. (Anna-Cajsa Torkelsson, Omni, Idag 12:24).

En fråga kommer då att ställas på sin spets: Skall besökare och de äldre bära munskydd?

I Sverige verkar vi verkligen ha duckat munskyddsfrågan, och informationen om dem är obefintlig. Alla har sin egen uppfattning om hur de fungerar. Man kan dock se fler och fler som bär dem.

Finland
Text-TV rapporterar idag att Finland kommer att spendera en miljard på att återuppbygga förråden inför en eventuell ny Corona-våg. Kommer något liknande att hända i Sverige? Min gissning är tyvärr nej.

Övrigt

LAS-frågan seglar nu upp som en stor fråga. Mycket olycklig tajming som jag ser det. Vi skulle kunna ha en elektronisk folkomröstning om lagen i höst och tillsvidare avföra den från agendan.

Today, the Minister for Social Affairs impressed by honesty. In morning-TV, she explained that "[...] none of us know if we are doing the right things." [freely translated] (Isabelle Nordström, Omni, Idag 10:41) This is probably one of the major truths presented during this crisis. I hope that the courage is politically rewarded.

The Testing Quagmire
The Regions seems to completely ignore their citizens who wants a test. To make everything worse, it seems like new actors are added to the mess. I briefly noticed that now also SKR [private organisation for regions and councils] seems to be involved.
Now blaming logistics is at the fore. I do, however, not see logistics as anything that could be blamed. There are three major ingredients, resources, will and ability. With these, logistical problems can be solved.

I have been getting the uncomfortable feeling that decisions makers have made testing depending on IT-development. I hope that I am wrong. Developing IT-systems takes valuable time. Some people do not realize that things can get done without an IT-system. Pen and paper, or Excel, can be used from the beginning. When the systems are at hand, early data can be entered. A demand for an IT-system is unfortunately a brilliant excuse for the "action phobiacs".


SWEDISH

Idag har socialministern imponerat genom ärlighet. Hon förklarade i morgon-TV att ”[…] ingen av oss vet om vi gör rätt”. (Isabelle Nordström, Omni, Idag 10:41) Det här är nog en av de större sanningarna som har yppats under krisen. Hoppas att det blir politisk belöning för modet.

Testhärdsmältan

Regionerna verkar framhärda i att bortse från sina innevånares önskan att bli testade. För att göra allt ännu värre verkar man tillföra nya kockar till soppan. Noterade som hastigast att även SKR verkar var insyltade. Nu skyller man på logistik. Jag anser dock inte att logistik är något man kan skylla på, det finns tre ingredienser, resurser, vilja och förmåga. Med dessa kan man lösa logistiska problem.

Jag har också fått en obehaglig känsla av att man gjort testverksamheten beroende av IT-utveckling. Hoppas jag har fel. IT-utveckling tar värdefull tid. Det finns människor som inte inser att man börja göra saker i praktiken utan IT-stöd. Till och med papper och penna räcker till att börja med, eller Excel. När systemen är klara kan man börja lägga in tidigt data. Ett krav på IT-utveckling är tyvärr en lysande ursäkt för dem med handlingsfobi.

A Certain Optimism (References, see the Swedish text)

Some articles on Omni today breath a certain optimism. Remdesivir seems to be useful, and it looks like progress is made on the concept of treating Corona. In another article, it was told that in the USA, planning is done for a 100,000 persons test of a vaccine.

Researcher Del Rio claimed that if 40 percent are vaccinated and 20 percent have been infected, we will have herd immunity.

The news gives me a brighter outlook on life. I am not one of the pessimists. The Dooms Day Prophets now claim that the world will never be the same. I think that, in a couple of years and for most people, the Corona will only be an unpleasant memory. The obvious exceptions are those who lost a relative or their company during the pandemic.

Further, I think that we will see a few changes in society. A certain increase in work from home might be one. If society, primarily the Government, gets better at handling crises remains to be seen. Here, I am unfortunately not really optimistic. A lot of reshuffling in the power structures are needed to achieve real change. The power structures are, however, very good at defending themselves. A large number of people get rich rewards from today's structures, and they will fight hard to keep them. It does not seem likely that any Swedish government in the near future will be able to tackle them. One could just imagine the defense mobilized if an abolition of the regions was suggested. Every close down of a less useful government authority would most likely cause an outcry.

SWEDISH

Viss optimism

Några artiklar på Omni under dagen andas en viss optimism. Remdesivir verkar göra nytta, och det ser ut som om konceptuella framsteg inom Coronavården görs. I en annan artikel redogjordes för att man i USA odlar en viss optimism runt att ha ett vaccin till årsskiftet. I USA planeras ett stort försöksprojekt med 100 000 personer. (Jan Velander, Omni Ekonomi, Idag 08:29, 08:28 och 07.37) Forskaren Del Rio menade att om 40 procent vaccineras och 20 procent har varit smittade så uppnår vi flockimmunitet. (Tyler Clifford, www.cnbc.com, May 22 2020, 5.09 PM EDT)

Nyheterna får mig att se ljusare på tillvaron. Jag tillhör för övrigt inte pessimisterna. Domedagsprofeterna säger nu att världen aldrig mer kommer att bli sig lik. Jag tror att Corona för de flesta endast kommer att vara ett obehagligt minne om ett par år. Undantagen är naturligtvis alla som förlorade en anhörig eller ett livsverk under pandemin.

Jag tror vidare att vi kommer att se några samhällsförändringar i Coronans spår. En viss ökning av hemarbete kan vara en. Om samhället, främst staten, blir bättre på att hantera kriser återstår att se. Här är jag tyvärr inte helt optimistisk. Man behöver möblera om rejält i maktstrukturerna för att få en verklig förändring. 

Maktstrukturerna är enormt bra på att försvara sig. Väldigt många får sitt rikliga levebröd från dagens strukturer och de kommer att kämpa hårt för att behålla sina positioner. Det verkar tveksamt om någon svensk regering i närtid kommer att rå på dem. Man kan bara fundera över det motstånd som kommer att mobiliseras om en nedläggning av regionerna kommer på tal. Varje nedläggning av en mindre nyttig myndighet kommer sannolikt också att mötas med ramaskrin.

Today's Good News
Region Sörmland has started with test-yourself-tests. The experiment is done in cooperation with the Authority for People's Health. (Anders Hovne, Omni, Idag 15:10)
This is good in itself, and for me, it is the first sign of the Authority for People's Health doing something concrete.

Today's "I do not know what"
A professor did today present the opinion that the number of dead per capita is not interesting. One would need to compare the figure to the spread of the disease. (Anders Hovne, Omni, Idag 15:56)
One question is how much we really know about the spread of the disease. The professor's fixing with figures feels like meaningless chauvinism. To discard a figure as "not interesting" also feels highly doubtful.

Dagens goda nyhet

Region Sörmland börjar med egentester. Försöket görs i samarbete med Folkhälsomyndigheten. (Anders Hovne, Omni, Idag 15:10) Detta är bra i sig, och för mig det första tecknet på att Folkhälsomyndigheten är inblandade i praktisk handling.

Dagens ”jag vet inte vad”

En professor gick idag ut med åsikten att döda per capita inte är intressant. Man behövde ställa siffran i relation till smittspridningen. (Anders Hovne, Omni, Idag 15:56)

En första fråga blir hur mycket vi egentligen vet om smittspridningen. Professorns sifferexercis känns som meningslös chauvinism. Att avfärda en siffra som ”inte intressant” känns också högst tveksamt.

Grades
One thought is on the top of my head: I wonder what grade that Löfvén (PM) would give to the Swedish handling of the Corona crisis so far?

Grades could also be handed out to different actors, for example on a scale of one to five, where five is the best. In order to this real fairness, one must have a better insight than I have. From my position I, however, dare to suggest the following grades:

-The Autority for People's Health: Grade 2 (test strategy and forecasts are on the deficit side, other efforts are unclear)
-The Authority for Social Affairs: Grade 1 (anybody who knows more maybe wants to raise)
-The Authority for Civilian Defense: Grade - (the only effort I have perceived is an app halfway completed)
-The Regions, health care: Grade 5
-The Regions, testing: Grade 2 (weak 2)
-Region Stockholm, face mask tests: Grade 1
-The Authority for Medical Drugs: Grade 2 (low visibility)

Sweden could also be ranked on a European scale, where the best is put on top. This type of comparisons is usually most popular when one is in a good position, and the opposite.
Europe today counts 45 nations. Will Sweden take place 22 or above? Will Slovakia take place one?

Several persons will probably claim that it is too early to make lists, but then one should switch critical thinking to "On". Why do they not want to evaluate now? An advantage with lists is that one can learn from the good examples. This knowledge could come in handy, especially as the virus might be back this fall.

Of course, the writings above reflect a certain amount of frustration, and steam needs to be let out. I do, however, think that grades would be good. If the Government made the critera for grades clear, more might be done.

Flock immunity is dead
Yesterday, Wednesday, flock immunity died when Tegnell declared it to be a myth. (Peter Dahlin, Omni, Igår [onsdag] 21:20.) It is good to keep a diary, then one can easily go back in time. On 11 April, I heard the word for the first time. Since then, it has been repeated several times, and I have even believed in it, although I have never completely understood it.
Flock immunity became 39 days of age. I will probably miss it a bit. It was kind of a nice idea. 

Other 
The Government has now addressed the matter of vaccination. One can observe that the actors and the strategy are the same as previously used for testing. Since the risk of food rationing now seems to have been just a scare, I will most likely focus on vaccination for some time. I really look forward to getting a vaccination.
Otherwise, have been offered an antibody test by a private company at 950 krona. Temptating. I have sent them an e-mail, asking which test they use, but still not received an answer.

SWEDISH

Betyg

En tanke ligger högst upp i mitt huvud: ”Undrar vilket betyg Löfvén så har långt sätter på Sveriges hantering av Corona-krisen som helhet?”

Betyg kan också delas ut till enskilda aktörer, på till exempel en skala ett till fem. Man måste ha bättre insikter än vad jag har för att kunna göra detta med riktig rättvisa. Från min utsiktspunkt dristar jag mig dock att komma med följande förslag.

-Folkhälsomyndigheten: Betyg 2 (teststrategin och prognoser tynger, insatserna i övrigt är otydliga)

-Socialstyrelsen: Betyg 1 (den som vet mer kanske vill höja)

-MSB: Betyg - (det enda jag har uppfattat är en halvfärdig app)

-Regionerna, vård: Betyg 5

-Regionerna, testning Betyg 2 (klart svag tvåa)

-Region Stockholm, munskyddstest, Betyg 1

-Läkemedelsverket: Betyg 2 (otydligt framträdande)

Sverige skulle också kunna inordnas på en europeisk skala, där man sätter den bäste överst. Den här typen av jämförelser brukar vara mycket intressanta när man ligger bra till, och tvärt om.

Europa består av 45 nationer. Tar sig Sverige över plats 22? Ligger Slovakien på första plats?

Många vill nog säga att det för tidigt att göra listor nu, men då gäller det applicera kritiskt tänkande. Varför vill de inte utvärdera nu? En fördel med att göra listor är att man lära sig av de goda exemplen. Denna kunskap kan vara bra att ha, särskilt som viruset kanske kommer tillbaka i höst.

Ovanstående speglar naturligtvis en viss frustration, som behöver släppas ut, men jag tror ändå att det vore bra med betyg. Om regeringen gjorde betygsskalan tydlig kanske det skulle hända mer.

Flockimmuniteten är död

Igår onsdag dog flockimmuniteten då Tegnell avfärdade tanken som en myt. (Peter Dahlin, Omni, Igår [onsdag] 21:20) Det är bra att föra dagbok, då kan man gå tillbaka. Den 11 april uppfattade jag för första gången begreppet. Sedan dess har det upprepats flera gånger och jag har till och med trott på det, även om jag inte riktigt har förstått begreppet.  

Flockimmuniteten blev 39 dagar gammal. Jag kommer nog att sakna den lite. Flockimmuniteten var en ganska trevlig tanke.

Övrigt

Regeringen har nu tagit tag i vaccinationsfrågan. Man kan notera att aktörerna och upplägget är samma som kring testningen. Eftersom risken för livsmedelsransonering verkar ha varit ett skrämskott kommer min uppmärksamhet sannolikt att i första hand gå till vaccineringen. Jag ser fram emot att bli vaccinerad. Har för övrigt fått ett propå från en privat aktör som vill sälja ett antikroppstest för 950:-. Frestande. Har mailat och frågat vilket test man använder, men ännu inte fått något svar.

Economy
My thoughts are usually not drawn to the economy. I am thinking that if it works out with the disease, also the economy will work out.
One news from Omni yesterday did, however, made me jump. "The State Budget in a 400 billion deficit this year." (Emma Hedin, Omni, Idag [200519] 09.52.)

The article suffers from the same problem as most all news. One never gets a figure to compare with, making the information difficult to evaluate. Neither a message from the Government Debt Office (19 maj 2020), which the article had a link to, contained any figure to compare with. There was, however, enough information to calculate that the Swedish GNP in 2019 was 5,059 billion krona, slightly more than I thought.

I believe that the State Budget is 1,100 billion krona, and then a deficit of 400 billion looks disastrous. It would be interesting to know how much is loss of revenues and how much is increase in costs, one day, one might get to know.
It is going to be interesting to see how this is solved. An increase in taxes might back-fire and printing or borrowing money will fuel inflation. I wonder if we have forgotten what a nuisance inflation is.

Slovakia 
I found the text below in a newsletter. There were a lot of interesting things in the newsletter, although it is difficult to evaluate most of it with no medical education. I did, however, feel that I wanted to add the following to my diary:

Lessons From Slovakia—Where Leaders Wear Masks (The Atlantic, 13 May) Perhaps the greatest lesson to be learned from Slovakia is of the value of leading from the front. * The country’s president, Zuzana Čaputová, and its nascent government administration, which came to power amid the pandemic, were among the first to model best practices for how to handle the outbreak—most notably by wearing protective masks and gloves for the government’s swearing-in ceremony. * Their efforts played a crucial role in normalizing the use of face masks across the country. * Despite often-conflicting advice about the efficacy of masks, Slovakia was among the first countries worldwide (and the second in Europe, after the Czech Republic) to make them mandatory in public spaces. * The picture in Slovakia has now changed—but its leaders are still proceeding with caution and leading by example.

Ekonomi

Mina tankar brukar inte dras till ekonomin. Jag tänker att om det löser sig med smittan så löser det sig med ekonomin också. En nyhet från Omni igår fick mig dock att rycka till: ”Staten väntas går back drygt 400 miljarder i år.” (Emma Hedin, Omni, Idag [200519] 09:52)

Artikeln lider av samma problem som i stort sett all nyhetsrapportering: Man får aldrig någon siffra att relatera till, varvid informationen blir svår att utvärdera. Inte heller det pressmeddelande från Riksgälden (19 maj 2020) som artikeln hade en länk till innehöll någon jämförelsesiffra. Det fanns dock tillräckligt med ledtrådar i pressmeddelandet för att man skulle kunna räkna ut att BNP år 2019 var 5 059 miljarder, något mer än vad jag trodde.

Jag tror nu att statsbudgeten är 1 100 miljarder, och då ser ett underskott på 400 miljarder närmast katastrofalt ut. Det vore intressant att veta hur mycket som är intäktsbortfall och hur mycket som är kostnadsökningar, men det kanske man får veta någon gång. Det skall också bli intressant att se vilket tänkande som de nya siffrorna leder till. En skattehöjning kan rekylera och att köra sedelpressarna eller låna sätter igång inflationen. Jag undrar om vi har glömt att det senare är ett otyg.

Slovakien

Hittade nedanstående ord i ett nyhetsbrev. Det fanns mycket intressant i nyhetsbrevet, även om det för en icke-medicinare är svårt att värdera det mesta. Kände dock att jag ville foga nedanstående till min dagbok.

Today, I am a bit flat. Quite tired of the Corona, but still concerned about not getting infected. The test quagmire is the matter engaging me most.

The Test Quagmire
Denmark and Great Britain seems to succeed in offering tests widely. The Swedish authorities have completely forgotten an important group, the public. Me and many others badly want a test, but it seems impossible to get one. Questions are met with demands for a letter from a doctor, and there it founders.
The authorities disregard an important part of human nature, one wants to know.

Risks in General 
The Corona and the forest fires in earlier years have shown what the politicians earlier have thought impossible "bad thing happens".

I came to think of one way of tackling the problem. The Government could list all bad things that could happen, with an emphasis on "could". The Government could then present this list on their homepage, and explain which risks they disregard och which they want to hedge for in some way. They should also develop what they, in concrete terms, are doing or wants to do in the cases where the risk is not disregarded.
Insurance business could most likely offer some inspiration in this process.

It then occurred to me that the Opposition could present their risk list. Then it would be up to the voters to value these lists in their choice of party to vote for.

The Next Wave
Another thought occurred to me when reflecting on risk. Day 49 might feel like too late to start doing things right. But there might be another wave of Corona this fall. I just hope that there is a planning process going on for handling a return of the Corona in the best possible way.

SWEDISH

Idag är jag lite avslagen, ganska trött på Corona men ändå angelägen om att inte bli smittad. Testhärdsmältan är det som engagerar mig mest.

Testhärdsmältan

Danmark och Storbritannien lyckas tydligen väl med att erbjuda en bred testverksamhet. De svenska myndigheterna har helt glömt en viktig grupp, allmänheten. Jag och många med mig vill verkligen få ett test, men det verkar omöjligt att få ett sådant. Förfrågningar möts med krav på remiss och där stupar det.

Myndigheterna bortser iskallt från en viktig del av den mänskliga naturen här, man vill veta.

Risker i största allmänhet

Corona och tidigare års skogsbränder har visat vad politikerna länge har tänkt bort, ”elände kan inträffa”.

Jag kom att tänka på ett angreppssätt. Regeringen skulle kunna lista allt elände som kan drabba oss, med ordet kan betonat. Regeringen skulle sedan kunna lägga upp denna lista på sin hemsida och förklara vilka risker som man bortser från och vilka man bör ta höjd för. Man borde även utveckla vad man konkret gör eller skall göra i de fall som man inte bortser från risken.

Försäkringsbranschen skulle förmodligen kunna ge en del inspiration i processen.

Min nästa tanke här var att det här är inte bara en fråga för regeringen. Här behövs ett bredare engagemang, och inte bara en förhoppning om att politikerna skall göra allt rätt. Olika organisationer torde ha en roll här. Regeringen skulle också kunna ha ett diskussionsforum på sin hemsida, och tillgodogöra sig alla de kloka synpunkter som med största säkerhet finns i samhället.

Oppositionen skulle kunna presentera sin risklista. Sedan skulle det bli upp till väljarna i nästa val att väga in dessa risklistor i sitt val av parti att rösta på.

Nästa våg

Ytterligare en tanke slog mig medan jag reflekterade kring risker. Dag 49 kan det börja kännas som för sent att göra saker och ting rätt. Men, det kan komma en Corona-retur i höst. Jag bara hoppas att det pågår en planering för att hantera en retur på bästa möjliga sätt.
