I looked at the first hour of the party-leader debate yesterday. Centern (formerly Farmer's Party) impressed me by stating that in times of crisis, one needs to think outside of the box.

The discussion on elderly care felt a bit confusing. Sometimes one got the impression that the State is hiring employees in elderly care. The way I have understood it, the State share in elderly care is legislative and surveillance through the government body IVO. As I understood it, neither of these two fields were discussed.

The Test Quagmire got justified attention, but, today I cannot explain what was being said and which concrete suggestions were made by the opposition.

One of the comments afterwards caught my attention. Lena Mellin said in Aftonbladet [Swedish newspaper] that some politicians seems to have had a crash course in epidemiology. (Louise Cassemar, Omni, Idag 08:58). I can only understand Mellin's irony as saying that an epidemic is a technical matter, not a political one. According to me, this stance is really dangerous.

Politicians have to make decisions and not hide behind science, which at times can be quite weak. Logically, politicians have to decide which science to trust, thus they can never duck. A recent example is how the Swedish Government has chosen to ignore the WHO call for face masks.

Mellin's writing cannot be seen as fake news. It does, however, have an undercurrent of "you do not understand this" - dangerous in a democratic society.

Yesterday, 26 researchers launched the idea that people living with an infected person should be quarantined. (Lisa Andersson, Omni, Igår 17:18) The idea seems quite reasonable, almost like one does not need to be a researcher to come up with it.

Tittade på första timman av partiledardebatten igår. Centern imponerade på mig genom påståendet att man måste tänka utanför boxen när det är kris.

Diskussionen om äldrevården kändes lite förvirrande. Ibland fick man intryck av att staten anställer människor i äldrevården. Som jag förstår det så är statens del i äldrevården lagstiftning och övervakning genom IVO. Inget av dessa två områden diskuterades, så vitt jag förstod.

Testhärdsmältan fick rättmätig uppmärksamhet, men jag skulle idag inte kunna förklara vad som egentligen sades och vilka konkreta förslag som lades från oppositionen.

En av kommentarerna efteråt fångade mitt intresse. Lena Mellin menade i Aftonbladet att några politiker verkar ha gått en snabbkurs i epidemiologi. (Louise Cassemar, Omni, Idag 08:58) Jag kan endast tolka Mellins ironi som att en epidemi är en teknisk fråga, inte en politisk. Enligt mig är den här inställningen livsfarlig.

Politiker måste fatta beslut och inte gömma sig bakom sakkunskapen, som ofta är nog så svag. Rent logiskt måste politikerna besluta sig för vilken sakkunskap man skall förlita sig på, så politikerna kan aldrig ducka. Ett aktuellt exempel är att regeringen väljer att bortse från WHO:s rekommendation om munskydd.

Mellins skrivning kan inte tolkas som ”fake news”. Den har dock en underton av ”det här begriper du inte” – farligt i ett demokratiskt samhälle.

Igår gick 26 forskare ut med att anhöriga till smittade borde sitta i karantän. (Lisa Andersson, Omni, Igår 17:18) Tanken verkar helt rimlig, nästan så att man inte borde behöva vara forskare för att komma på den.

Something Fishy around the Face Masks

After yesterday's blogging, I saw a couple of statements from Tegnell (freely translated):

- Face masks do not fit the Swedish Corona-strategy.

- "One always has to be careful with WHO statements. One cannot select a sentence, one has to see the whole and then form an opinion on what they actually are saying."

These statements generate a lot of follow-up questions, but there seems to something fishy here. The questions are: "What fish and where?"
I wonder if Tegnell is under some sort of political pressure, saying: "No face masks - never no way!"

I also wonder if we have the key in our hand, but refuse to use it. Successful nations like Slovakia has been using face masks extensively.

If the travel recommendation had been combined with a call for face masks, I should have felt a lot more comfortable. Now I see it as a great gamble.

The Test Quagmire
The Senior MD and professor in clinical bacteriology Agnes Wold said concerning increased tracking of infections to SvD [Swedish newspaper] that [freely translated]: "I do not think that it is doable today." She further claimed that the number of newly infected is about 1,000 a day.

Wold strengthens my idea that the major division line between people in this world runs between optimists and pessimists. Another important divider is the one between "It-will-never-work"-people and "Let-us-get-started"-people.
It would be interesting to hear a discussion on infection tracking between Wold and for example a few Chinese who work in the field.

One could also wonder which evidence there are for 1,000 newly infected a day. Something that could be published?

Något är märkligt med munskydden

Efter gårdagens bloggning såg jag några uttalanden från Tegnell:

- Munskydd passar inte in i den svenska Corona-strategin.

- ”Man måste alltid vara väldigt försiktig med WHO:s uttalanden. Man kan inte plocka ut en mening utan måste se på helheten och göra en bedömning av vad de egentligen säger.”

Man skulle kunna ställa många följdfrågor på dessa uttalanden, men det verkar ligga en hund begraven här. Frågan är: Vilken hund och var? Jag undrar om Tegnell är under någon sorts politiskt tryck som säger: ”Inga munskydd, inte till något pris!” Jag undrar också om vi har nyckeln i handen, men vägrar använda den. Framgångsländer som Slovakien har varit ettriga med munskydd.

Om resebeskedet hade kombinerats med en rekommendation om munskydd hade jag känt mig betydligt mer bekväm med det. Nu ser jag det endast som ett stort vågspel.

Testhärdsmältan

Överläkaren och professorn i klinisk bakteriologi Agnes Wold sade angående utökad smittspårning till SvD att: ”Jag tror inte det görbart idag.” Hon menade vidare att antalet nysmittade ligger runt 1 000 om dagen. (Louise Cassemar, Omni, lördag 14:57)

Wold stärker min uppfattning om att den viktigaste skiljelinjen i världen går mellan optimister och pessimister. En annan viktig skiljelinje är ”Det-går-inte-människor” kontra ”Nu-sätter-vi-igång-människor”. Det vore vidare intressant att höra en diskussion mellan Wold och till exempel några kineser som är insatta i frågan.

För övrigt kan man undra vad det finns för evidens för att vi har runt 1 000 nysmittade om dagen. Något som går att publicera?

Late yesterday evening, I saw on Text-TV that WHO now recommends face masks. I feel a bit satisfied, having been a few days ahead of WHO here. It remains to be seen how the Swedish Government reacts to the recommendation.

The question about the Government revived my old question: "Who governs?". I do carry a feeling the Government has abdicated in favor of the Authority for People's Health. There is a party leader debate tomorrow. Normally, I do not watch these debates, but I feel that I must make an exception tomorrow. One could always learn something.

The Test Quagmire
In practice, there seems to be no news regarding the quagmire. What strikes me is that offcial Sweden seems to have no sense of urgency. Everyone seems to slowly walk in their ordinary tracks, not letting themselves be disturbed by the need for testing.

One way of solving the problem would be to invite a Chinese team. According to the text below, they could do Sweden in around ten days.

There might be many explanations to the Swedish problems. The lack of clear political signals could be one. I wonder how clear political signals today has to be to generate action. Testing is one example, the Police another.

Sent igår såg jag på Text-TV att WHO nu rekommenderar munskydd. Jag känner mig lite belåten med att ha varit ett antal dagar före WHO på den punkten. Nu återstår att se hur den svenska regeringen reagerar på rekommendationen.

Frågan om regeringen återuppväckte min gamla undran: ”Vem regerar Sverige?” Jag går ofta omkring med en känsla av att regeringen har abdikerat till förmån för Folkhälsomyndigheten. Normalt sett vill jag inte titta på partiledardebatter, men jag måste nog göra ett undantag i morgon. Man kanske blir klokare på något.

Det ser inte ut att vara några direkta nyheter i praktiken rörande härdsmältan. Det som slår mig är att offentligheten verkar sakna krismedvetande helt. Man verkar lunka på i sina byråkratiska hjulspår utan att låta sig störas för mycket av testbehovet.

Ett sätt att lösa testproblemet är att bjuda hit ett kinesiskt team. Enligt texten nedan klarar de Sverige på drygt tio dagar.

De svenska problemen kan ha många förklaringar, bristen på tydliga politiska signaler kan vara en. Jag frågar mig hur tydliga politiska signaler som idag krävs för att få något gjort. Testningen är ett exempel, polisen ett annat.

Today's blogging gets early and long. I simply do not have much to do at the moment.

The Test Quagmire
The Government has now provided funds for testing. Somewhere, I think that I have seen that private companies are lining up to get their share of these funds. In a way, that is good. The pace of the operation can now increase.
It did, however, occur to me that if the Government/regions open the wallet without quality control, we might have another scandal ahead of us.

Another idea around testing is that I think that the decision makers are possessed by a mean idea of rationing, connected to narrowly defined utility for society. This idea results in the construction of rules and this heavy administration makes all flat.
The contrasting idea would be the generous one - test everyone who wants to be tested, fast and simply, but collecting relevant statistics.
My idea is not completely developed, but I think that there is a lot to be found in "Corona-psychology" and the decision makers' mind-set.

Finances
The main labor union (LO) now suggests that the financial support to companies during the crisis shall result in higher taxes on incomes from capital and on fortunes. They also suggest that the State should take shares in return for support. (Jon Velander, Omni Ekonomi, Onsdag 06.58)

The first part looks like pure populism. Raising taxes is blunt and it lowers economic growth, and thereby employment. If growth is desirable, the company tax should be set at zero percent, and taxes on income from capital should be really low. Again, I came to think of one of my favorite writers, Paul J. Feldstein, who often pointed out the danger in politicians seldom or never stating the goals for their actions.

The second part looks a lot better. This is a precision measure, which forces the companies to think twice before they line up for taxpayer's money. The companies receiving support can then re-purchase their shares from the Government. The taxpayers do, however, probably have to count on some total losses in the process.

Professors
There are many professors (head of institutions in Sweden) surfacing in media, and I have since long felt a need to comment on Swedish professors. I meet a lot of people who have a strong confidence in professors. The way I see it, this is not completely justified. For starters, there has been a considerable inflation in professor's title. When I counted last time, there were 113 professors in Government Studies. For Sweden, three ought to be reasonable figure. With the increase in volume, one could assume a dilution in competence.

Second, professors are often inclined to say what politicians want to hear. Third, "Beta-professors" tend to follow "Alpha-professors". If Alpha-professors are wrong, the herd tends to follow anyway. Fourth, professors do not always agree among themselves. Thus, there must be a number of professors being wrong.
Fifth, there are professors in topics which are not scientific in any respect, for example that there are hypothesizes which could be tested or there is an ability to explain and predict.

 Therefore, critical thinking should not be disconnected just because a professor is saying something. When several professors agree, it gets better, a high degree of intersubjectivity. One can in these cases not, however, exclude the possibility that several professors are sensitive to what the politicians might want to hear or follow an Alpha-professor. 

Other

The Police in Malmö broke up a demonstration which constituted a breach of Corona-rules. This was a victory for Swedish society, and Swedish society needs victories. In Stockholm, society celebrated no triumphs.

Det blir en lång och tidig bloggning idag. Jag har helt enkelt inte så mycket att göra för tillfället.

Testhärdsmältan

Regeringen har nu anslagit pengar. Tycker mig ha sett någonstans att ett stort antal privata företag är på jakt efter en del av kakan. Det är på ett sätt bra, det kan då bli annan fart på verksamheten. Det slog mig dock att om regeringen/regionerna börjar fördela pengar utan att ha en kvalitetsuppföljning så kan vi få ytterligare en skandal.

En annan tanke kring testningen har slagit mig. Jag tror att beslutsfattarna är besatta av en snål ”ransoneringstanke” med koppling till en snävt definierad samhällsnytta. Tanken leder till att man bygger upp snåriga regler kring testningen och allt blir platt.

En motsatt mental inställning skulle vara den generösa – testa alla som vill bli testade, snabbt och enkelt, men med insamling av relevant statistik.

Min tanke är inte helt utvecklad, men jag tror att det finns mycket att hämta i ”Corona-psykologin” och beslutsfattarnas mentala inställning.

Ekonomi

LO menar nu att företagsstödet skall leda till högre skatter på kapitalinkomster och förmögenheter och att staten borde ta en ägarandel i de företag man stödjer. (Jon Velander, Omni Ekonomi, Onsdag 06.58)

Den första delen verkar vara ren populism. Man föreslår en trubbig åtgärd som sänker tillväxten och därmed antalet arbetstillfällen. Om man vill ha tillväxt bör företagsskatten sättas till noll procent, och skatten på kapitalinkomster bör vara riktigt låg. Jag kommer åter att tänka på en av mina favoritförfattare, Paul J. Feldstein, som ofta påtalade faran med att politikerna sällan eller aldrig kan förklara vad de har för mål för sina förslag.

Den andra delen verkar betydligt bättre. En precisionsåtgärd som tvingar företagen att tänka två gånger innan de börjar trängas vid köttgrytan. De företag som behöver stöd kan sedan köpa tillbaka aktierna från staten. Skattebetalarna lär dock få räkna med en hel del totalförluster.

Professorer

Det dyker upp många professorer i media och jag har länge känt ett behov av att skriva av mig i frågan. Jag träffar många som tror att professorer alltid har rätt. Som jag ser det är detta inte helt berättigat. Till att började med så är professorsinflationen kraftig. Senast jag räknade fanns det 113 professorer i statskunskap, tre borde vara en lämplig siffra för Sverige. Med det ökade antalet kan man anta en uttunning i kompetens.

För det andra är professorer ofta lyhörda för vad politiken vill höra. För det tredje är ”beta-professorer” benägna att följa ”alfa-professorer”. Om alfa-professorer har fel följer flocken gärna med. För det fjärde är professorer inte alltid överens. Det måste alltså finnas ett antal professorer som har fel. För det femte finns det professorer i ämnen som inte är vetenskapliga i någon bemärkelse, till exempel att det finns hypoteser som kan testas eller att man kan förklara och förutse.

Man bör alltså inte koppla bort det kritiska tänkandet för att en professor säger något. När flera professorer är överens är det lite bättre, hög intersubjektivitet. Man kan i dessa fall dock inte utesluta att flera professorer är politiskt lyhörda eller följer en alfa-professor.

Övrigt

Polisen i Malmö löste upp en Corona-vidrig demonstration. Detta var en seger för samhället, och samhället behöver segrar. I Stockholm firade samhället inga triumfer.

It has earlier occurred to that the armed forces could have done more in the Swedish Corona-crisis. British writer Christopher Brannigan is fielding the same ideas:

“Operation COVID-19 argues that mandarins should learn from military planning practices, which proved to be so successful in the building of the Nightingale Hospitals, repatriating British nationals and setting up mobile testing stations.

Lessons learned from the Armed Forces would help to address the powerlessness of departments and ministries to deliver on policies that so often depend on outside agencies for their execution.

They would also, Chris Brannigan argues, address the Civil Service’s “excessive numbers of meetings” and the “absence of common terminology” that leads to misunderstandings and delays.”

The Swedish Defense Forces might not be in the proper shape to do something. Most likely, they could, however, borrow higher management staff from the UK. This could be beneficial in many ways.

Elderly care
Some discussions on elderly care are alive. Among others, Johan Fischer had a letter to the editor just published. I thought that it really was worth considering:

"The so called "Ädel" reform of 1992, forbidding counties to hire medical doctors, is the reason why the Corona has spread in elderly care. [...]
Medical competence, both MD:s and nurses, has to take actual charge and as in hospital care, lead, develop, train and correct. We need to reform everything, top down. It is most likely hard work and takes time. It is needed." [Freely translated] (se Swedish for reference)

Swedish society seems unable to evaluate reforms. I did at once come to think of Enquist's psychiatric reform [not much appreciated reform in the middle of the 1990s].

Fischer meant that it most likely will be a lot of work to correct Swedish elderly care. One way to simplify could be to do as one does in the computer world. One removes the latest version and go back to the old, in which known errors are fixed.

The Test Quagmire
The Government now reserves 5.9 billion krona for testing (Text-TV), which I think is good. It then remains to be seen if this money can be converted to action, which is the difficult part of the process.
It shall also be interesting to see if useful statistics are produced from the testing activities. This has a lot to do with Brannigan's words above.

Eventuellt är den svenska försvarsmakten inte i form att göra något. Man skulle dock sannolikt få låna några chefer från Storbritannien. Det kanske skulle vara nyttigt på flera sätt.

Äldreomsorgen

En del diskussioner om äldreomsorgen lever, bland annat har Johan Fischer gjort ett inlägg som jag tyckte var tänkvärt.

”Den så kallade ädelreformen 1992 som förbjöd kommuner att anställa läkare är orsaken till att coronaviruset spridit sig i äldreomsorgen. […]

Medicinsk kompetens, både läkare och sjuksköterskor, måste få ta det konkreta kommandot och som sjukvården leda, utveckla, utbilda och rätta till. Det svenska samhället verkar vara dåligt på att utvärdera reformer. Jag kom direkt att tänka på Enquists psykreform.

Fischer menade att det säkert blir arbetsamt att rätta till reformerna. Ett sätt att förenkla är möjligtvis att göra som i datavärlden. Man tar bort den senaste versionen och återgår till den gamla, i vilken man rättar kända fel.

Testhärdsmältan

Regeringen avsätter nu 5,9 miljarder till testning (Text-TV), vilket jag tycker är bra. Det återstår sedan att se om man kan omvandla dessa pengar till konkret handling, vilket är det svåra i processen. Det skall också bli intressant att se om man kan få fram användbar statistik ur testverksamheten. Det här har väldigt mycket att göra med Brannigans ord ovan.

3 June, Wednesday, Day 64

Sympathy for Tegnell 

Today, I sympathize with Tegnell. There is a blame game going on, with the PM on the Minister for Social Affairs and the Minister for Social Affairs on Tegnell.

I have asked myself what has been happening. This is my theory: The Government did not know what to do, at all. Doing what science said seemed like a safe strategy, and one that could not evoke critique afterwards.

The Government asked science, in the shape of a few persons whom they could trust. Science, in the shape of the trusted persons, made the mistake to deliver on the expectations. They tried to look reassuring, instead of providing facts, assumptions and uncertainty, and then leave it over to the politicians to make decisions on these grounds. The politicians could escape responsibility and Tegnell carries the burden.

The most interesting part in process seems to be the meetings with epidemiologists from all regions which the Authority for People's Health obviously are arranging. These meeting are probably the most important factor in the struggle against Corona. They ought to be televised in real time. Then everyone could get an uncensored picture of the limits for our knowledge, and what the, on the matter, most likely, best-informed people think.

General - Coordinator 

The Government "Test General", actually a coordinator, now resigns after three weeks in office. (Anders Hovne, Omni, 02 juni). The resignation is not surprising. A general is a person with a task and resources. A coordinator is a person who quickly resigns.

Idag sympatiserar jag med Tegnell. Det har varit ”katten på råttan och råttan på repet”, eller Löfvén på Hallengren och Hallengren på Tegnell.

Jag frågade mig vad som egentligen har hänt. Jag tror så här: Regeringen visste inte vad man skull göra, alls. Att göra som vetenskapen sade verkade vara en säker strategi, och en som inte skulle kunna kritiseras.

Man frågade vetenskapen, i form av några personer som man kunde lita på.

Vetenskapen, i form av de personer som regeringen kunde lita på, gjorde dock misstaget att svara upp mot förväntningarna. De försökte inge säkerhet istället för att påtala fakta, antaganden och osäkerhet, för att sedan överlåta besluten till politikerna på denna grundval. Politikerna kunde därmed krypa ur sitt ansvar och Tegnell blir skuldbelagd.

Det intressantaste inslaget i problemkomplexet är de möten som Folkhälsomyndigheten verkar ha haft med immunologer från alla regioner. Dessa möten är förmodligen de viktigaste i hela kampen mot Corona. De borde direktsändas i TV för att alla skall få en ocensurerad bild av var gränsen för okunskapen går och hur vindarna blåser bland dem som kan antas vara de mest insatta i sakfrågan.

General – Koordinator

Regeringens ”testgeneral”, egentligen koordinator, avgår efter tre veckor i befattningen (Anders Hovne, Omni, 02 juni). Avgången är föga förvånande. En general är en person med en uppgift och med resurser. En koordinator är en person som avgår efter en kort tid.

Today's blogging has to be short. Omni just reported on a study of social distancing. In less than a meter, the risk of contamination is 13 percent. It is then halved for each meter up to three. (Olivia Wikström, Omni, 15:02) Good news, but I wonder if they also apply indoors. The article in Science that I looked at the other day did not give that impression.

Face masks - Again

I cannot stop thinking about face masks. I planned to get a few for myself, the local apothecary wanted SEK 499 for 50 of them. More mean than self-preserving, I did not take the offer and I am now looking for a better price.

Suddenly, a graph appeared in my head, see below. I am sure it can be improved, but the basic idea is that better protection would call for less closing down of society. What if the face masks are extremely good for the economy, especially in combination with fever pistols? Could we have winded up on a not optimal place in the graph?

Dagens bloggning måste bli kort. Omni rapporterade nyss om en studie om social distansering. Smittrisken på mindre än en meter är 13 procent, och halveras med varje meter upp till tre. (Olivia Wikström, Omni, 15:02) Bra nyheter, men jag undrar om siffrorna gäller även inomhus. Den artikel i Science som jag kikade i för ett par dagar sedan gav inte den känslan.

Munskydden - igen 

Kan inte sluta tänka på munskydden. Tänkte skaffa mig några själv, men det lokala apoteket ville ha 499:- för 50 stycken. Snålheten slog självbevarelsedriften, och jag skall hålla utkik efter lägre pris.

Plötsligt dök en graf upp i mitt huvud, se nedan. Den går säkert att förbättra, men grundtanken är att bättre skydd skulle kunna minska nedstängningen. Tänk om munskydden skulle vara extremt bra för ekonomin, kanske i kombination med feberpistoler? Tänk om vi har hamnat på en helt inoptimal punkt?
